Mount Hermon Play School Registration Forms

For New students: you need an application as well as the following four forms. You also need to send a check for $150 which includes the registration fee and the deposit.
These documents are necessary for you to read, but I do not need them signed or brought back to me.
For returning students, all you need is a new application and the $100 deposit unless you let it forward to this year. I need you to update your emergency information and you can take care of this at the parent meeting or at the "visit" day in September.
Concerning Financial Help
Dear Parents,
Requests for financial help will be considered for no more than half of the tuition, and only for the 9 to noon option. For pre-K, scholarships are only available for 3rd year students or children age 5 by September 1st. More information and instructions are on the application, so to apply, download the form and return it by the end of August. Please send me an email or give me a call to let me know that you have mailed it, so I can know to look for it.

What families qualify for help?
Well, where do I start? Since we depend on the money from the tuition payments and what ever is available in the scholorship fund to run our program, the families that are not paying for any other programs for their child or paying for any other “fun” things due to stressed finances are what this is for.
How much is available?
In May, 2009, play school families helped us raise a little over $5,000 at our Silent Auction. That was our last fund raiser and we now depend upon people donating to the fund. Please feel free to share this opportunity to give to our program with your friends and family. Thanks
